Subscription Terms

Effective Date: July 1, 2026

Automatic-renewal disclosure. Any therapy subscription or other recurring purchase you elect (including any recurring lab plan or recurring visit plan) will automatically renew at the price and cadence shown at checkout until you cancel. Single-purchase items are not subscriptions and do not automatically renew. You can cancel any recurring purchase at any time as described below. By enrolling in a recurring purchase, you authorize Leader Health and our payment processor to charge your payment method on a recurring basis.


Compliance framework. These Subscription Terms are designed to comply with the federal Restore Online Shoppers' Confidence Act (15 U.S.C. §8401 et seq.), applicable state automatic-renewal laws (including, by way of example and not limitation, California Business & Professions Code §17602 (as amended by AB 2863, effective July 1, 2025), New York General Business Law §527-a (final amendments effective November 5, 2025), Oregon Revised Statutes §§646A.293 and 646A.295, the Colorado Online Cancellation Act (SB 25-145, effective August 6, 2025 for consumer subscriptions), the Illinois Automatic Renewal Reform Act, and the equivalents in other states whose automatic-renewal laws are then in effect), and the Federal Trade Commission's guidance on negative-option marketing. These Subscription Terms are drafted to the strictest-common-denominator of the applicable state automatic-renewal laws and are applied uniformly to all patients. The FTC's "Click-to-Cancel" rule, formally the Negative Option Rule (16 C.F.R. Part 425), was vacated by the United States Court of Appeals for the Eighth Circuit on July 8, 2025 and is not currently in effect; these Subscription Terms incorporate the practices the FTC's guidance and the state laws above require, and will be updated if the rule is reinstated, modified, or replaced.

Creating a Leader Health account is free. These Subscription Terms govern recurring purchases you elect through the Service. A "Subscription" is any recurring purchase you enroll in, including:

  • Therapy subscriptions (recurring medication or therapy billed at the cadence shown at checkout).

  • Recurring lab plans (a monitoring panel scheduled to recur on a defined cadence).

  • Recurring Provider-visit plans (a follow-up visit scheduled to recur on a defined cadence).


Single-purchase items — not subscriptions. Single-purchase items (one-time products or medications, one-off lab panels, or one-off visits) are not Subscriptions and do not automatically renew. They are governed by the refund and return terms shown to you at purchase and are not covered by these Subscription Terms.


Therapy-specific minimum commitments. Minimum commitment, if any, is therapy-specific and is disclosed at checkout for that therapy. Some therapies are month-to-month; some carry a disclosed minimum term (for example, a 3-month minimum). There is no blanket platform-wide minimum and no membership minimum. Where a specific therapy carries a minimum term, the total minimum amount you are committing to over the minimum term, the recurring price, and the renewal cadence are disclosed to you in dollars and cents on the checkout screen for that therapy, and you must affirmatively acknowledge that disclosure to complete the purchase.

Creating a Leader Health account is free. These Subscription Terms govern recurring purchases you elect through the Service. A "Subscription" is any recurring purchase you enroll in, including:

At checkout, for each recurring purchase, you see, prominently and before you pay:

  • The full Subscription name and what is included.

  • The recurring price (in dollars and cents).

  • The cadence (for example, monthly, every 3 months, or annual).

  • The therapy-specific minimum commitment, if any, and the total minimum-term dollar amount that figure represents.

  • When you will be charged and on what payment method.

  • How to cancel.

a. Billing cycle. Therapy subscriptions are billed in advance of each refill ship date or on the cadence shown at checkout. Recurring lab and recurring visit plans are billed in advance of each scheduled order or visit.


b. Automatic renewal. Subscriptions renew automatically at the same price and cadence unless you cancel before the next billing date. Annual-cadence Subscriptions renew on the anniversary of the start of the current term.


c. Annual renewal reminder for plans of one year or longer. For Subscriptions with an initial term or renewal term of one (1) year or longer (including any annual-cadence therapy, lab, or visit plan), Leader Health will send you a renewal reminder notice between 15 and 45 days before each automatic renewal of that term. The reminder will be delivered to the email address (and, where required by state law, by text message or postal mail to the contact information) on file for your account, and will include (i) that your Subscription will automatically renew, (ii) the renewal date, (iii) the renewal price, (iv) the length of the renewal term, and (v) how to cancel before the renewal charge. This notice is intended to satisfy California Business & Professions Code §17602(c), New York General Business Law §527-a, and similar state automatic-renewal reminder requirements then in effect. Oregon's automatic-renewal statute (ORS §§646A.293 and 646A.295) does not impose an annual pre-renewal reminder duty, and this Section 3(c) is not offered as satisfying an Oregon-specific reminder requirement; Oregon's disclosure and consent requirements are addressed in Section 2 and Section 3(a)–(b) above. We deliver this notice to all patients on Subscriptions of one year or longer, regardless of state of residence, so that the strictest-common-denominator standard is applied uniformly.


d. Price changes. We may change Subscription prices. If a price change applies to your Subscription, we will notify you by email at least 30 days before the change. The new price applies on your next renewal after the notice period. You may cancel before the new price takes effect.


e. Failed payments. If a charge fails, we may retry, attempt to contact you, and suspend access pending resolution. Sustained failure to pay may result in account closure.

You can cancel any Subscription at any time using the same method by which you signed up. Cancellation does not require speaking to a representative and does not require steps or screens more burdensome than signup.


Online (primary method). Sign in → Account → Subscriptions → Cancel. Confirmation is shown immediately on screen and emailed to you.

Other methods, equally available:
Email: support@myleaderhealth.com (subject: "Cancel Subscription") — we confirm by reply within 24 hours.

Mail: Leader Health, c/o LH Ventures LLC, 321 S Persimmon St, Tomball, TX 77375, Attn: Subscriptions.

For any recurring purchase, cancellation is effective at the end of the current billing cycle, subject to any therapy-specific minimum term disclosed at the checkout for that purchase, as further described in Refunds & Cancellations.

You may pause any active recurring purchase for up to 90 days per pause, twice per year, by request to support@myleaderhealth.com. Pause stops billing for the paused purchase; messaging and lab-ordering access tied to that purchase are suspended during the pause.

If we offer a trial, the offer terms — duration, what's included, what you'll be billed after, and when — will be disclosed prominently at signup before you agree. You will be charged at the end of any trial unless you cancel before the trial ends.

By clicking the "Subscribe," "Start therapy," "Start plan," or similar button to enroll in a recurring purchase, you (a) confirm you have read and agree to these Subscription Terms, (b) authorize recurring charges at the price and cadence disclosed at checkout, and (c) agree to receive electronic communications regarding your Subscription. We retain a record of your subscription consent, the price, cadence, any therapy-specific minimum term, and the disclosure shown to you.
